Presenting Problem
Kurt is a 27 years old male who contact our agency requesting help for his Bipolar Disorder also known as Manic Depression Disorder. He stated that he had been having Bipolar or Manic Depression for many years. As a child, he was diagnosis with ADD (Attention Deficit Disorder). He never got the right treatment for these diagnoses.
Currently Kurt describes his depression as hard to manage, irritability, and distractibility causing him to have inability to work and loss interested in social activities with his family and friends.
Client would like to be on medication to help him cope with his depression. He would like to do social activities with his wife and daughter again and doesn’t want his disorder to be a disturbance to them. He is compliance with talking to his doctor and would accept any treatment given to him.
Family of Origin
Client has a sister name Kim who was born in 1971 and at times he would spend time with her. His parents often argue. Client describes his parents as always yelling and fighting at each other. Both children had to deal with it. Parents got divorce when he was eight and he had to go back and forth with visitation. Both parents got remarried. Father remarried to his stepmother Jenny and her two children. He believes that father always takes his stepmother side and favored her children and his half brother Chad. Mother remarried to Pat ‘O Conner, who she divorce later because he was abusive. At times when Kurt was visiting his mother she usually is drunk and yells at him.
Client has a history of substance abuse, which is alcoholism that runs in his family and two suicides in the Cobain family, common in males. Age 12, client’s great-uncle Burle, which is the brother to Kurt’s grandfather shoot himself in the stomach with a pistol and another great-uncle a year later who die also with shooting a pistol. Then another grand uncle falls off the stairs while intoxicated.
Kurt spent his time living with friends when he could. He also slept in apartment hallways and in hospital waiting room. He doesn’t have a place to live.
Birth and Childhood
Pregnancy and Birth was not discussed due to client not knowing the information. Client had a happy childhood until he was eight years old, when his parents got divorce. Before the divorce, Kurt had interest in art and music.
He also learned to play the piano by ear and enjoys a kiddie drum kit his parent gave him. His father urges him to play little league baseball. After the divorce, client became withdrawn. He lived with his father and visits his mother and sister on weekends.
Client is unhappy at home and stated that he never felt loved and secure. Kurt was a trouble maker when he was 12 years old. Client began experimenting with drugs to self medication.
Marriage and Significant Relationships
Kurt married his wife for three years. They have one daughter who is three years old. Kurt’s wife was pregnant with their daughter when they met in 1992 during their concert trips. Kurt describes his wife as unpopular and using him to get famous. She also was using drugs during the pregnancy. At times they often fought about drug use.
Current Living Arrangements
Client lives with his wife and daughter in 5,000 square feet three story house in Seattle with a value of $4,535,000.00. The house has 4 bedrooms and 5 bathrooms. They also bought a $1.13 million mansion. Kurt stated he often uses drugs and that sometime led arguments between his wife and him, so he moved into a cheap $18 a night hotel.
Education
Client never finishes high school. He dropped out of Aberdeen High School due to not having enough credits to graduate. Before Kurt high school drop out, he enjoys drawing during classes and joined a wrestling team. He also has interest in art and music.
Military Service
There was no military Service.
Employment History
After his drop out Kurt mother gave him a choice, either to find work or leave. He decided to leave and left home. Kurt worked at a Polynesian coastal resort. In 1989, became the leader of a band called Nirvana.
Medical History
Aside from Kurt’s manic depression and ADD, Kurt suffered from chronic bronchitis and physical pain in the stomach. Kurt stated using marijuana in 1980 at age 13 and was using it on a regular basic. He also had a period of consuming LSD and used heroin. Beside the drugs, Kurt also drinks alcohol. Client also has a family history of suicide and mental illness in the Cobain family. In 1992, Kurt did attempt his first rehab and after he found out he was going to have a daughter he left rehab.
Legal History
In July 1985, Kurt was arrested for spray painting in town with some friends. His friends got away but he was caught and taken to the police station. The next year he was in trouble with the law after being found wandering around an abandoned building being drunk at night. He end up spending several days in jail.
Social and Recreational Interests
Kurt has interest in art and music. He spent a lot of his time exploring different creative outlets such as writing, painting, drawing, and making collages. He also has talent as a songwriter. Since childhood Kurt was withdrawn and feels misfit in his family.
Religious Activities
Kurt lived with his father who is a born again Christian. Kurt became a devout Christian and attended church services regularly. Religion remains an important part of his personal life and beliefs. Often he uses Christian imagery in his work and maintained interest in Jainism and Buddhist philosophy.
Client Successes, Strengths, and Resources
Client is a successful songwriter and is in a band. He has talent in art and music that is his biggest strengths.
Client did not share any information on resources or support. However, his wife is also there when he tries to commit suicide.
Impressions and Recommendations
This is a 27 years old male who was diagnosis with bipolar or manic depression disorder. He often has thoughts of suicide. He also did not feel that he receive the love and security he needed from his parents. He never got the treatment needed for the diagnosis and self medicate by using drugs and drinking.
My recommendation to Kurt would be taking 10 counseling sessions with a therapist working on his poor self-esteem and family issues. Also having an evaluation done for treatment on his diagnosis determining if he needed medication or other treatment. The last recommendation is spirituality play, helping him with dealing with his problems.
